High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed a threaded hose Bibb located on front service area with hose connected without a back flow device connected.

Basic - Exterior door has a gap at the threshold that opens to the outside. Observed light shining from outside at the exit door near rest rooms near door handle.

Basic - Floor tiles missing and/or in disrepair. Observed floor tiles next to walk in cooler door in disrepair. Repeat Violation

Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed walk in cooler concrete floor not sealed.

High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw shell eggs stored over ready to eat mayo. Manager removed and stored properly. Corrected On-Site

Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed concrete floor of walk in cooler unsealed.
