Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles in prep area soiled with flour and dust near vents.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed refrigeration fan in walk in cooler dusty.

High Priority - Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee engaging in food preparation touch apron and proceeded to touch equipment. Inspector intervened and employee removed glove and washed hands. Corrective Action Taken

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed on JAM bar Half and Half (47F-56F - Cold Holding). Container was filled to the top. Manager placed in at 7am. Manager removed and placed in walk in cooler and replaced with smaller amount. Corrected On-Site

Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee drink on prep table located on cook line. Employee removed and stored properly. Corrected On-Site

Intermediate - Spray bottle containing toxic substance not labeled. Observed spry bottle containing bleach without label. Manager placed label on spray bottle. Corrected On-Site

High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed Chlorine Dish machine at 0ppm. Chemical bucket was empty , manager had employee replace during inspection. Took second test at 100ppm. Corrected On-Site
