Basic - Water leaking from backflow preventer on dish machine. Manager called in work order during inspection.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach-in cooler on end of make line for 2 hours: potatoes au gratin 45f, mozzarella 44f. Operator put potatoes in ice bath and brought them to 41f in 30 minutes. Mozzarella was voluntarily disposed of. Corrected On-Site

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling vents throughout kitchen are soiled with mold-like substance. Per manager, cleaning vents is on this week's agenda.

High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Raw tuna in walk-in cooler baring label to be removed from reduced oxygen environment before thawing completely thawed without being removed from reduced oxygen environment.

Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package.
