Basic - Employee drinking from an unenclosed beverage container in a food preparation/clean dishware area and the container does not have a dispensing mechanism to prevent hand contact with lip contact area of drink container. Observed one employee drink with no lid or straw on prep table. Person in charge discarded. Corrected On-Site

Basic - Floors walls, and/or ceilings soiled/has accumulation of debris. 1.) Observed vents in back kitchen with buildup of black mold like substance. 2.) Floor under cookline with heavy buildup of grease. 3.) Standing water on floor in corner by mop sink. Standing water in chicken walk in cooler.

High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 1 live flying insect at the time of inspection.

Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No test kit available at the time of inspection.

Basic - Ceiling tile missing. 2 ceiling tiles missing in dining room.
