Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up on the pipes in the walk in freezer. Repeat Violation

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets being soiled with food debris on the reach in freezer on the cook line.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles towards the bathroom area soiled with dust.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up on the pipes and on the floor in the walk in freezer.

Basic - Missing drain plug at dumpster. Repeat Violation

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the exterior of the microwave soiled with food debris. Manager told employee to clean the microwave exterior. Employee cleansed the exterior of the microwave. Corrected On-Site

Basic - Opened employee beverage container in a cold holding unit with food to be served to customers. Observed an employee drink on a shelf in the reach in freezer. Manager removed drink. Corrected On-Site

Basic - Water leaking from pipe and/or faucet/handle. Observed a leaking faucet in the mop sink area.

High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed one live fly inside of plastic lid next to soda machine in lobby. Manager threw lid away, fly no longer visible. Corrected On-Site

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Creamer found on top of reach in cooler. at room. Temperature at 49F. Manager voluntarily discarded creamer. Corrective Action Taken

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed soda nozzles next to pick up window with black substance inside. Corrected On-Site

Intermediate - No running cold water at hand sink. Observed on cookline.. Manager turned on cold water at the sink. Corrected On-Site

Basic - Backflow device not located for convenient service or maintenance access. Observed two missing black flow devices. One needed in extra faucet and the other needed on right hose extending from the splitter.

Basic - Floor area(s) covered with standing water. Observed water standing on floor near cook line.

Basic - Floor soiled/has accumulation of debris. Observed grease on floor next to hot water heater.

Basic - Missing drain plug at dumpster.

Basic - Water leaking from pipe and/or faucet/handle. Observed leaking faucet on hand washing sink located next to cook line.

Intermediate - Handwash sink not accessible for employee use at all times. Observed dustpans stored in front of the handsink. Manager moved the dustpans. Corrected On-Site

Basic - Food stored on floor. Observed soda stored on the floor in the storeroom. Manager put the soda on the shelf. Corrected On-Site

Basic - Unwashed fruits/vegetables stored over ready-to-eat food. Observed tomatoes stored above shredded lettuce in the walk-in cooler. Manager moved the tomatoes to the bottom shelf. Corrected On-Site

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed the paper towels were empty in the machine. Manager restocked the paper towels. Corrected On-Site

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the gaskets on the walk-in cooler with debris and mold like substance on the gasket.

Basic - Open dumpster lid. Observed the dumpster lid open.
