High Priority - Live, small flying insects found. Observed approximately 8 small flying insects in the following locations: 5 in kitchen, 3 in dry storage. Warning

High Priority - Raw sewage on ground of establishment. Observed raw sewage overflowing from urinal in men's bathroom. Warning

High Priority - Sewage/wastewater backing up through floor drains. Observed raw sewage backed up from floor drain in kitchen between double door reach in cooler and make table and in women's bathroom. Warning

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles and vents with an accumulation of dust and grease in kitchen area. Also observed ceiling tile with black mold-like substance in mop sink area. Repeat Violation

Basic - Cove molding at floor/wall juncture broken/missing. Observed broken and missing cove molding near charbroiler. Repeat Violation

Basic - Exterior door has a gap at the threshold that opens to the outside. Observed hole/gap at threshold of exterior back door. Repeat Violation
