Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at women's employee bathroom handwash sink. Operator called maintenance to turn up the hot water. Corrective Action Taken

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Ice machine in rear of kitchen has accumulation of mold-like substance in far back of bin.
