Observed no signed copy of Employee health reporting policy, printed a copy and had all employees sign. Corrected On-Site

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed building up of ice in the reach in freezer next to the office desk in the rear prep kitchen.
