Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at rear kitchen hand sink. Operator replaced towels. Corrected On-Site

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed food prep employee with gold bracelet on.

Basic - Reach-in freezer interior/shelves have accumulation of soil residues. Observed food debris buildup on bottom of reach in freezer.

Basic - Employee with ineffective hair restraint while engaging in food preparation. Observed one employee who was wearing a hat but their hair was down to the middle of their back and worn in the front and back. Operator asked the employee to pull their hair back with another hair restraint. Corrected On-Site

Basic - Employee with no beard guard/restraint while engaging in food preparation. Observed three employees with facial hair long enough to require a beard guard and were not wearing one. Operator provided employees with masks to use as a beard restraint. Operator stated they will order proper beard guards. Corrected On-Site

Basic - Observed cut out openings in the ceiling tiles located above fryer area, above syrup rack, and above tankless water heaters in kitchen. Operator stated they will put in work order to close the openings. Corrective Action Taken
