High Priority - Tracking powder pesticide used inside establishment. Small amount of tracking power pesticide under the equipment in the drive through area. Manager started the powder will be removed after closing tonight.

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.

Basic - Floor soiled/has accumulation of debris. Floors under all equipment in the kitchen soiled.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Soda lines on the bag in box soda machine next to the walk in cooler soiled.

Basic - Unclean building components, attachments or fixtures. Fan covers in the walk in cooler soiled.

Basic - shelves with rust that has pitted the surface. Observed front storage room shelf legs pitted with rust.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup in walk in freezer. Repeat Violation

High Priority - Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed an employee wipe her nose then grab a to-go container. Operator had the employee wash their hands. Corrective Action Taken

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed the operator helping on the cook line while wearing a watch. Instructed them on the proper food code.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup on the door of the walk in freezer preventing it from closing fully.

Intermediate - Water treatment device has not been inspected or serviced according to manufacturer's instructions. Observed no dates on water filters by soda rack.

Basic - Shelves with rust that has pitted the surface. Obviously shelves in front storage room rust pitted.

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed one food employee with gold bracelet on wrist. Employee removed bracelet. Corrected On-Site

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed wet stacked pans above three part sink. Repeat Violation

Basic - In-use tongs stored on equipment door handle between uses. Observed tongs on heated cabinet door handle. Operator removed. Corrected On-Site Repeat Violation

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed clean food containers with chili buildup. Operator removed and put in sink. Corrective Action Taken

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed stacked wet pans on shelf above three part sink. Operator spread pans out to dry. Corrective Action Taken

Basic - Floor soiled/has accumulation of debris. Observed floor by reach in freezer and floor under fryer heavily soiled and buildup of debris.

Basic - In-use tongs stored on equipment door handle between uses. Observed tongs on cabinet drawer handle on cook line. Operator removed. Corrected On-Site

Basic - Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Observed standing water by mop sink.

Basic - Floor soiled/has accumulation of debris located behind soda machine in dining room. As a repeat violation the importance of following proper protocol with this violation was reinforced with the operator. Repeat Violation

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed soda machine in dining area to be soiled with food debris. Operator cleaned the nozzle. Corrected On-Site

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed three ceiling tiles in dining room with water damage.

Basic - Floor soiled/has accumulation of debris. Observed grease on floor under fryers in kitchen.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed ice machine exterior soiled above hinge. Operator cleaned exterior of ice machine. Corrected On-Site
