High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Observed pizza dough with water from leak in ceiling on dough and in container that was sitting on a prep table behind the oven. Operator discarded.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed leak in ceiling in kitchen next to the oven.

Basic - Floor area(s) covered with standing water by the oven from leak in ceiling.

Basic - Objectionable odors in bathroom or other areas of the establishment in the rear of the kitchen in the warewashing area. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee. Repeat Violation
