Basic - Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed quaternary sanitizer bucket in dining room at 0 ppm. Operator changed quaternary sanitizer. Quaternary sanitizer bucket now at 200ppm. Corrected On-Site

Basic - Floor tiles missing and/or in disrepair. Observed floor tiles missing and cracked under and around fryers.

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at handwashing sink across from fryers. Operator replaced paper towel. Corrected On-Site

Basic - Bowl or other container with no handle used to dispense food. Observed solo cup being used as a scoop in blue cheese. Operator removed cup. Corrected On-Site

Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed open drink on shelf in food prep area next to strainer. Operator discarded drinks. Corrected On-Site

High Priority - Toxic substance/chemical improperly stored. Spray bottle of cleaner observed on bread rack. Manager removed spray bottle. Corrected On-Site

Intermediate - No soap provided at handwash sink. Hand sanitizer was observed in men's restroom, manager stated they were waiting for a new soap dispenser. Soap dispenser arrived by the end of inspection and was installed. Corrected On-Site
