Although the operator is using time as a public health control, no tracking record was available. Form was provided to the operator during the inspection.

Observed accumulation of black mold like substance on the underside of soda machine dispenser nozzle located at lobby area. And also observed accumulation of black like substance on the underside of the smoothie machine dispenser nozzle located inside kitchen prep area.
