High Priority - Non-food grade paper/paper towel used as liner for food container. Observed non food grade paper towels wrapping raw salmon and raw tuna. Also paper towels lining various cut vegetables.

High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw salmon and tuna being stored on top of cooked eel. Located in front reach in cooler.

Basic - Bowl or other container with no handle used to dispense food. Observed bowl with no handle being stored in container of rice and flour.

Basic - Ceiling tile missing. Located above prep table(downstairs).

Basic - Clean pots and pans not stored inverted or in a protected manner.

Basic - Commercially processed reduced oxygen packaged tuna bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package.

Basic - Food stored on floor. Observed container of food being stored on floor in walk in cooler.

Located in reach in cooler under front counter. Observed raw beef being stored on top cooked rice in walk in cooler. Observed raw salmon being stored on top of cut lettuce in kitchen reach in cooler.
