Basic - Equipment and utensils not properly air-dried - wet nesting. Observed food metal pans and plastic pizza pans not properly air dried and wet nesting.

Basic - Floor not cleaned when the least amount of food is exposed. Observed the floor in between the prep table and freezer unit, in the back area soiled with grease and food.

Basic - Garbage on the ground and/or pad around dumpster.

Basic - Missing drain plug at dumpster.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the gasket of the Walk in freezer door soiled. Observed gaskets of the freezer units located across 3 compartment sink soiled.

Basic - Observed back exterior door has a gap at the threshold that opens to the outside. Repeat Violation

Basic - Observed grease on the ground pad around grease receptacle.

Basic - Working containers of food removed from original container not identified by common name. Observed a pump container located on a prep table with no label.

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed no vacuum breaker at mop sink. Coached operator on vacuum fitting and application.

Intermediate - Employee with long fingernails working with exposed food, single-service items or clean equipment and utensils. Observed employee not wearing gloves with long painted nails while engaged in food preparation.

Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed containers being stored on bottom shelf not inverted in dish area. Operator inverted containers. Corrected On-Site

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee wearing watch engaged in food preparation.

Basic - Exterior door has a gap at the threshold that opens to the outside. Observed gap at back door.

Basic - No Heimlich maneuver/choking sign posted. Observed no choking poster on site. Provided choking poster via email to operator. Corrected On-Site

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Air vents soiled at kitchen next prep table and by office.

Basic - Food stored on floor. Observed a case of oil stored on floor next to upright freezer. Manager had it stored 6" above floor. Corrected On-Site

Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed inside microwave soiled by back door.

Basic - Old labels stuck to food containers after cleaning. Observed old labels on ready to use container at three compartment sink. Manager removed the label. Corrected On-Site Repeat Violation
