Observed employees Sally, Luis, and Nelly without employee training. As per operator employees were hired 3-6 months ago.

Intermediate - Spray bottle containing toxic substance not labeled. Observed spray bottle with sanitizer not labeled at hand sink next to pizza prep area.

Basic - No handwashing sign provided at a hand sink used by food employees. Observed no hand wash sign at hand sink next to pizza prep area.

High Priority - Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed food employee touch garbage can and then touched clean pizza dough containers.

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink.

Intermediate - No soap provided at handwash sink. Observed at employee bathroom. Operator replenished soap.
