High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Upon inspection, observed sliced American cheese 57f and shredded cheddar 60f on make line in kitchen. Manager stated less than 30 minutes. Manager had employee move both sliced American cheese and shredded cheddar to colder holding unit for cooling. Corrective Action Taken

Basic - Dead roaches on premises. Upon inspection, observed one dead roach in rear storage area of establishment. Manager had employee remove dead roach during inspection. Employee also cleaned and sanitized area during inspection. Corrective Action Taken Warning

Basic - Exterior door has a gap at the threshold that opens to the outside. Upon inspection, observed rear door of establishment to have a gap between door and threshold. Daylight could be seen coming through the gap.
