High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. In back prep area, 1 flying insect observed. Also, in service area, observed 1 flying insect land on single service drink lids. Operator killed fly and discarded lids. Corrective Action Taken

Intermediate - Spray bottle containing toxic substance not labeled. By back door near office, pump sprayer unlabeled identified by manager as degreaser, manager labeled. Corrected On-Site

Basic - Dead roaches on premises. In service area of kitchen by pass through window, approximately 16 dead roaches located in electrical breaker box. Manager cleaned up. Corrected On-Site Warning

Basic - Floors, walls, and/or ceilings soiled/has accumulation of debris. In dish area, under large conveyor dish machine, food debris and solid buildup.

Basic - Grease on the ground. In back enclosed area, at outside mop sink, floor/drain area is soiled with grease.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. On cook line, across from fry station, Reach in cooler has significant grease and food debris on side of unit. In prep area, pans stored under table soiled with food debris. On top of steamer, oven gloves stored on top were soiled and damp. On cook line, across from chargrill, backside of Reach in cooler is soiled and has black mold like substance.

Basic - Plumbing fixture (toilet, urinal, sink) not easily cleanable. In kitchen, back prep area, hand sink faucet is corroded and no longer easily cleanable.

Basic - Standing water in bottom of reach-in-cooler. At bar, glass chiller, bottom of Reach in cooler has standing water.

Basic - Standing water in floor drain/floor drain draining very slowly. In back kitchen, under steamer, floor drain is not draining, some standing water present.
