Multiple employee drinks, in unapproved containers (bottles), were stored in the prep area and the walk in cooler. Employee drinks must be in single service cups with a lid and a straw. The drinks must be stored in an approved location away from food, equipment and utensils.

Employee, working with food, was observed with a watch. Employees may only wear a plain band/ring.

The inspection report was posted behind the counter. The inspection report must be posted no more than 15 feet from the door. 5 to 7 feet from the floor and within 1 foot approach.

The floors, walls and ceiling are unclean with dust and food debris. The floors, walls and ceiling must be cleaned more often to prevent the accumulation of dust and food debris.

Observed staff drinks in unapproved containers (water bottle, diet Dr pepper bottle) in food prep areas. Staff drinks must be in single service cups with a lid and straw and be stored below work area, or be stored out of food service areas. Bottles removed.

Observed food prep staff with watches, bracelets and jewelry other then smooth metallic band preparing food. Food staff may only have smooth metallic band type ring on hands/arms. Corrected.

Observed pizza make line staff and dough prep staff without proper and effective hair restraints. Ensure all staff handling food utilize proper and effective hair restraints.

Observed floors, walls ceilings in WIC, pizza line, dough prep, storage and dish line areas with accumulations of grease, oil, dust, splash, spills and/or debris. Clean surfaces at a frequency to prevent accumulations by 11/5/23
