Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed in all rooms dressers with sticky substance on top of them and some were either chipped or stained. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

Observed missing lightbulbs in rooms 106, 109 and 202. All lighting fixture in bathrooms need covers.

EHS observed the following:
room 103- dead insects in drawers and on the sheets, room 102-dead insect in the window seal. room-113-dead insect on the window seal. room 114-dead insects on the ac unit. room 215-live roach on mattress sheets. room 207-dead insect (fly) on the ceiling. room 203- live roach on the wall. room 106- dead roach under table.
CA- PIC needs to inform housekeeping if they see any insects dead or alive they need to properly discard them. For the rooms that had live insects PIC needs to inform pest control to spray those room extra until the problem is fixed.
511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program

EHS observed in all rooms that the dressers were chipped and stained. The top of the dresser had sticky substances on the top of it. All night stands in each room were damaged. Also, in room 106 the front of the ac unit was damaged. In rooms 203 the floor in needed to be cleaned. In room 112 the refrigerator needed to be clean. Lastly, In all rooms the air filters needed to be cleaned.
CA- PIC needs to make sure that housekeeping is cleaning all air filters and furniture in the rooms. Also if any furniture is damaged PIC needs to make sure they properly discard it and replace it.

CA- PIC needs to make sure that housekeeping is changing all beds in each room with clean sheets and mattress pads and also none of the sheets and mattress pads need to be damaged. If they are damaged or severely stained PIC needs to inform housekeeping to discarded and replace them.
