8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
EHS observed no gas-fired equipment inspection available for view at time of inspection. PIC indicated that the facility has had no inspection.
511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency
CA: Informed PIC that the gas-fired equipment inspection is required annually. Advised PIC that the facility is to have an inspection performed immediately by a qualified licensed contractor or local fire authority
17 - proper ventilation; no sign of mold, mildew, odors or moisture
Regulation: 511-6-2-.08(5) - toilet and bathing ventilation
EHS observed none of the bathroom ventilation systems to be working in all rooms inspected (101, 103, 105, 107, 111, 113, 115, 114).
511-6-2-.08(5) - Toilet and Bathing Ventilation Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems.

Regulation: 511-6-2-.12(2) - garbage and refuse containers
EHS observed the outside garbage container to be missing the right-side lid.
511-6-2-.12(2) - Garbage and Refuse Containers All outside refuse or garbage storage containers shall be constructed of durable metal or other approved types of materials, which do not leak and do not absorb liquids and shall be provided with tight-fitting lids or covers and shall be kept covered when stored. Each container shall be located within 100 feet of dwelling units or trailer spaces or in a location approved by the Health Authority and shall be cleaned at such frequency as to prevent a nuisance or odor.
CA: Advised PIC that the garbage containers should have a lid, and to reach out to the garbage service company for repair or replacement
