8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
EHS observed no gas-fired equipment inspection available for view at time of inspection. The PIC indicated that the report is stored in the main office, to which he does not have access.
511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency
CA: Advised PIC that all gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority. Recommended that a copy of the inspection report is stored in a location, available to front desk associates when the manager is not available
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation
Regulation: 511-6-2-.14(5) - condition of furnishings
EHS observed rusting and/or dirty microwaves in rooms 214 and 307.
511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.
CA: Advised PIC that appliances are to be kept in good repair. The observed microwaves must be repaired to original condition or replaced
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation
EHS observed missing lamp shades in rooms 209 and 306.

CA: Advised PIC that the lamp shades should be replaced
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered
EHS observed the following:
- Rm 204 - stains on sheets, mattress pads
- Rm 206 - Stains and hairs on mattress pad
- Rm 209 - Stains and hair on mattress pad
- Rm 214 - Stains and hairs on mattress pad
- Rm 306 - Hairs on sheets, stains on mattress
- Rm 305 - Stains on mattress pad
34 - grounds, walkways, porches and hallways maintained; drives provide unobstructed width and proper spacing; graded to drain; trailer spaces marked clearly, areas clean and maintained in good repair
Regulation: 511-6-2-.18(2) - grounds free of litter
EHS observed non-functional and/or damaged appliances (washer/dryers, ice machine, lamp bases), furniture (dressers), and A/C units stored on the main and 3rd floor walkways.
511-6-2-.18(2) - Grounds Free of Litter Grounds, including spaces beneath buildings and trailers, shall be kept clean and free of litter.
CA: Informed PIC that the grounds of the facility are to be kept free of litter, and non-functional and/or damaged items should not be stored in walkways
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair
Regulation: 511-6-2-.08(4) - toilet facilities-cleaning and maintenance
EHS observed the walls of the tubs/showers in rooms 204, 206, 209, 214, and 307 to be dirty. Also, in room 307 and 214 the covers to the wall ventilation system were missing.
511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists.
CA: Informed the PIC that bathrooms shall be kept clean and advised that the cleaning frequency is increased
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation

EHS observed the walls in all rooms but room 305 to be dirty and stained.
511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored.
