EHS observed mixer valve for sanitizer in 3C sink not providing sanitizer when basin is filled.
Equipment shall be maintained in a state of repair.
PIC to have valve repared/replaced.

EHS observed metal flashing on moulding by mop sink corner protruding, exposing glue/insulation beneath.
All physical facilities shall be maintained in good repair.

Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.

Violation: Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. P Inspection note: EHS observed front refrigerator not cold holding adequately (48F). Items out of temp include milk (45F, 47F) Corrective action: PIC immediately discarded TCS items and scheduled for maintenance later in the evening.

Violation: 1. Dressing rooms shall be used by employees if the employees regularly change their clothes in the establishment. 2. Lockers or other suitable facilities shall be used for the orderly storage of employee clothing and other possessions. Inspector note: EHS observed employees using storage areas not designated for personal items being used. Corrective action: PIC removed items.
