EHS observed the front interior surface of the microwaves in rooms 204 and 207 to be rusting, the paint peeling, and generally in need of maintanance.

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

CA: Advised PIC that all appliances are to be maintained in good repair; observed microwaves must either be repaired to original finish or replaced

EHS observed the a/c unit filter in room 109 to be dirty and the filter in room 204 to be damaged, in need of replacemant.

CA: Advised PIC to ensure housekeeping is regularly cleaning the a/c filters to prevent dust accumulation and filters are to be discarded and replaced when visible damaged

EHS observed the following:

• Room 104 - stains on top layer of linen
• Room 106 - hole in pillow cover
• Room 109 - hairs on sheets
• Room 204 - stains on mattress pad
• Room 207 - stains on sheets and mattress pad

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

CA: Advised PIC that the observed linen items with stains or hairs should be cleaned and replaced. If stains persists on linen items after repeated washes, facility is to discard and replace those items

EHS observed dirty ail filters in AC unit in all rooms observed. Violation: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. CA: clean air filters at a frequency to stop the accumulation of dust

EHS observed dirty ail filters in AC unit in all rooms observed. Violation: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. CA: clean air filters at a frequency to stop the accumulation of dust

EHS observed objectionable odors in all rooms observed. Violation: room ventilation shall be able to remove objectionable odors and provide comfortable living conditions. CA: as no other means of ventilation available in rooms wall units for AC should be turned on routinely to allow air to circulate in rooms

EHS observed facility unable to provide annual Gas Fired Equipment inspection. Violation: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency CA: have inspection conducted and/or find previous report. Send EHS report and keep on file for future insepctions

EHS observed observed curtains in rooms 105, 204, 206, 207,and 221 with stains and damage to curtains. Violation: all draperies must be maintained in good repair, clean, and free of vermin

EHS observed burn holes in covers rooms 204 and 206. Room 221 had debris on bedding. Violation: bed coverings shall be clean and free from vermin. CA: clean or discard linens. Burned linens shall be discarded

EHS observed mold like accumulations on showers in rooms 207 and 221. Violation: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. CA: clean or replace shower curtains

EHS observed objectionable odors in rooms 204 and 206. Violation: room ventilation shall be able to remove objectionable odors and provide comfortable living conditions. CA: as no other means of ventilation available in rooms wall units for AC should be left running to circulate air

EHS observed in the following rooms:

105: Detritus on inner curtains, stains. 205: Detritus on inner curtains. 206: Stains on inner curtains. 221: Detritus on inner curtains, stains.

EHS advised PIC to increase cleaning frequency of curtain liners, especially in smoking rooms

EHS observed the following:

Room 105: Hair on sheets between covers, detritus on pillowcases Room 221: Detritus on mattress pad, stains on mattress pad, stains on pillowcases Room 212: Hair on sheets, stains on pillowcases.

EHS instructed PIC to increase frequency of linen washing or to discard indelibly stained linens

EHS observed dirty air filters in rooms 104, 204, and 208. Also, in rooms 119 and 206 there were no air filters in the ac unit. In addition to in rooms 119, 206 and 208 the microwaves were dirty. In room 208 the fridge also was dirty.

Corrective action- PIC needs to ensure that housekeeping is cleaning each microwaves and refrigerators and making sure they are cleaning the air filters as well.

EHS observed In the following rooms:

Room 119- Stain on mattress sheets and mattress pad. Room 104- Hair on mattress pad. Room 206- Dirt on the mattress sheets and pest on the mattress sheet also. Room 208- Stained sheet and mattress pad.

Corrective action- PIC needs to ensure that housekeeping are properly washing each sheet and mattress pads. Once they make the beds in the clean rooms that they double check to make sure the sheets and mattress pads are free from dirt and hair.

EHS observed that the facility did not have the most recent inspection score posted. The inspection that the facility had posted was from 8/12/2022. The facility last inspection was on 1/5/2023.

Corrective action- PIC needs to make sure that the facility has the most recent inspection score posted.

511-6-2-.05(7) - Inspection Posting Any tourist accommodation inspection report addendum(s), completed by the Health Authority to document observations, violations, and corrective actions resulting from an inspection need not be displayed, but must be made available by the tourist accommodation operator to the public upon request
