EHS observed the hot water to be above 120F in all rooms, temps as follows:

• Room 105 (124)
• Room 115 (125)
• Room 121 (125)

511-6-2-.07(2) - Hot Water Supply Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F.

CA: Advised PIC that water temperatures in guest's rooms are not to exceed 120F, and recommended boiler temperature is lowered

EHS observed that the gas-fired equipment inspection could not be provided at the time of inspection. PIC indicated that he believed no inspection was required as their water heater/ boiler operated below 199BTUs.

511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

CA: Advised PIC that gas-fire equipment inspections are required annually by a licensed contractor or local fire authority if the facility operates gas-fired equipment

EHS observed most recent inspection report not to be on display. The inspection on display present a grade of 89, however the most recent inspection performed on 02/20/25 resulted in a grade of 78.

511-6-2-.05(7) - Inspection Posting The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away.

CA: Advised PIC that the most recent TA inspection must be made available for public view, and the facility is to post the TA inspection after an inspection is performed

EHS observed a housekeeping cart containing two spray bottles used for cleaning with no labels.

511-6-2-.17(2) - Housekeeping Carts Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label.

CA: Advised PIC that spray bottles used in cleaning are to labeled with it's current contents

EHS observed hot water as follow 104(126F), 108(124F), 212(123F), and 222(123F). Violation: Hot water in all guest rooms shall not exceed 120F. CA: ensure boiler is set at temperature that will not exceed 120 in guest rooms

EHS observed that facility could not provide gas-fired equipment inspection. Violation: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority. CA: have a licensed contractor complete annual inspection of all gas fired equipment

EHS observed the following furnishings in need of cleaning. Air Filters need cleaning: 104 and 108. Stained couch in 108. Violation: all furnishings shall be kept clean, in good repair, and vermin free. CA: clean all furnishings at rate that precludes accumulation of dust and debris. Replace furnishings that cannot be cleaned any longer

EHS observed stained bedding in 104, 108, 212, 215. Violation: mattresses and all bedding shall be kept clean and free of vermin. CA: ensure stains are removed in wash. If stains cannot be removed then replace bedding

EHS observed facility unable to provide pest control records for rooms. Log showed Orkin was out last month. Violation: A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. CA: keep invoices/reports where can be provided upon health authority arrival

EHS observed objectionable odor in rooms 108, 212, 215, and 222. Violation: rooms shall be ventilated and capable of removing objectionable odors. CA: allow wall unit ACs to operate and provide ventilation

EHS observed the following rooms water temperatures exceeding 120 degrees:

Room 104- water-126 Room 122- water-125 Room 208-water-121 Room 203-water-124

CA- PIC needs to inform maintenance about adjusting the water heater. The water has to be between 100-120 degrees.

EHS observed the following in each room:

Room 104- stained mattress pad and sheets. Room 122- stained mattress sheets. Room 120- Stained mattress pad and hair on sheets. Room 208-burn mark on blanket that was placed over the bed.

CA- PIC needs to inform housekeeping to make sure they are changing the mattress with clean sheets and pads that are free from debris. Any sheets that are damaged or stains will not come out must be properly discarded and replaced.

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

EHS observed the permit not being posted in the facility. EHS requested to see hotel permit. PIC was unable to located the permit.

CA- PIC needs to make sure the hotel permit is displayed on the property.

511-6-2-.04(1)a - Permit Display Any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority. (b) To qualify for a permit, an applicant shall:

1. Be an owner of the tourist accommodation;
2. Allow access to the tourist accommodation;
3. Provide all information as required on the application and pay all applicable local and state fees referenced in DPH Rule 511-6-2-.22 at the time the application is submitted and;
4. Score 100% on the permitting inspection for newly constructed facilities

Violation: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Inspector note: EHS observed bed linens with varying degrees of holes, stains and hair in rooms 123, 202, and 207. Corrective action: EHS instructed PIC to have linens with excessive holes replaced, linens with stains/hairs cleaned, and advised for housekeeping to wear hair restraints
