EHS observed the following in each room:

Room 418-hair on mattress pad Room 414-hair and stain on mattress pad. Room 309-stained mattress pad.

CA-PIC needs to make sure that housekeeping is cleaning all rooms with the clean bedding items. Any stained or damage items must be properly discarded and replaced.

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

EHS observe all air filters in each rooms were severely dusty.

CA-PIC needs to make sure that housekeeping is routinely cleaning the air filters in the rooms.

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

EHS requested for pest control records. PIC provided a sign in log but no report.

CA-PIC needs to make sure that facility is getting updated pest control records.

511-6-2-.13(2) - Insect and Rodent Control-Records A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. The record may be made by the permit holder or provided by the applicator, and shall list the following information: (a) amount and concentration of product used; (b) name of product used; (c) date and location of application; (d) application method; (e) pest targeted; and (f) name of applicator

EHS observed the the need for the microwave to be cleaned in the inside of the unit. There was stains still present inside of the unit. The rooms are 206, 208, and 323.

CA-PIC needs to make sure that housekeeping is routinely cleaning each microwave in each room to reduce stains and food debris from appearing.

EHS observed the following in each room:

Room 205-stained pillowcase and mattress pad. Room 121-Hair on the mattress pad Room 316-stain on mattress pad Room 320-stain on mattress pad Room 323-Stain on pillowcase, sheets, and mattress pad

CA-PIC needs to make sure that housekeeping is changing all bedding items with clean ones and making sure that the items are free from any stains, hair, or debris. If bedding items are damaged items must be properly discarded and replaced with new ones.

EHS observed water exceeding 120F in rooms 101, 204, 203, 301, 303, 409.

In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F.

EHS advised PIC to reduce temperature of boilers. PIC stated that she intends to have maintenance have a look at boiler functionality to ensure that water does not become too hot in future

EHS observed live insects that appeared to be bed bugs in room 303 crawling across pillowcases.

Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises.

EHS advised PIC to contact their pest control company, Arrow, and implement a control procedure to ensure insects do not spread. PIC stated that pest control would be contacted that day to start control

EHS observed water exceeding 120F in rooms 307, 314, 302, 311, 318, 320, 404, 408.

Violation Code Reference 511-6-2-.07(2) - Hot Water Supply Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F.

EHS advised PIC to reduce water temperature of boilers/heaters in order to maintain a maximum temperature of 120F
