511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists.
Corrective action: Discussed with PIC to have mirrors replaced, and to clean shower corners and to do minor caulking work repair in the rooms noted

511-6-2-.08(5) - Toilet and Bathing Ventilation Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems.

EHS observed stained sheets in the rooms 205 and 208.
CA-EHS showed PIC the stains sheets. EHS questioned. PIC about what do they do when sheets are stained/damaged. She stated they discard if the stains do not come out.

Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

EHS observed stained pillowcase on far right of bed in room 320.
EHS observed hairball on sheet below comforter on corner in room 327.
Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin.
EHS observed general aging including faint stains and wear of mattresses in all rooms observed.
EHS advised PIC to replace damaged mattresses. PIC stated that mattresses were in process of being replaced in facility. EHS also advised that post-drying linens should be inspected for hairball accumulation

Regulation: 511-6-2-.07(2) - water supply and sanitizing
EHS observed hot water exceeding 120 degrees Fahrenheit in rooms 219, 223, 227, 233, 224, and 308.
Corrective action: PIC needs to ensure that water should not be exceeding 120 degrees Fahrenheit. Adjust water heater or boiler to 120F additionally can place temperature regulation devices on faucet end.
511-6-2-.07(2) - Water Supply and Sanitizing Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 1200F

EHS observed debris and stains in the following rooms:
Room 203- mattress pad has a blood stain on it.
Room 223- hair on the mattress sheets.
Room 227- hair on pillow case and mattress had stained orange substance around the rim of the mattress.
Room 307: stain on mattress sheets.
Corrective action: PIC needs to ensure that housekeeping are changing sheets regularly and also before they mark a room "clean" make sure the mattress pads and sheets are free from stains and hair.
