EHS observed the incorrect Tourist Accommodation inspection report (grade-92, date-11/21/24) posted for display; additionally, the inspection report is displayed around a corner near the manager's office — a position not easily viewable.

511-6-2-.05(7) - Inspection Posting The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away.

CA: EHS advised the PIC that the inspection report shall be prominently displayed in public view at all times, and in a public area. Recommended the report is moved to the registration desk or displayed near the front entrance where it can be read at a distance of one foot away

EHS observed dark colored, mold-like substances on the interior ice dispensing door/flap in ice machines on floors 2, 4, and 7 which require cleaning.

511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice

EHS observed the ice machine on floor 6 to be leaking, a non-insignificant amount of water was observed on the floor in the ice machine room.

CA: EHS advised the PIC that ice machines must be maintained in good repair. The observed machine should be repaired to stop the water leak

EHS observed bedding and/or mattresses stained, damaged, or with debris in rooms: 808, 716, 733, 624, 513, 432, 210, 205, 218, 407, 623, 708. Violation: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly. CA: ensure that all bedding and mattress are cleaned and kept in good repair. May need to invest in waterproof covers to avoid staining of mattresses

EHS observed inspection report posted in corner by management offices where could not be easily seen. Upon closer look inspection report posted was for Food Service at the main kitchen. Violation: The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away. CA: post most recent inspection report and will need to where can be seen. Recommended to be posted near front service desk

EHS observed chipping, staining, and mold like build up in tubs of rooms 819, 716, 624, 335, 205, 218, 407, 619, 826, 708, 623. Violation: all surfaces in bathrooms shall be maintained clean and in good repair. CA: ensure that cleaning showers at a frequency to prevent accumulations and staining, repair or replace liners that are chipped

Violation: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Inspector note: EHS observed facility unable to provide annual gas-fired equipment report. Corrective action: PIC will make the inspection report available upon request either by written copy or digital copy via plumber. Facility uses Watson Plumbing for gas-fired equipment report

Violation: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. Inspector note: EHS observed varying degree of debris accumulation in 419, 417, 421, 511, 823,826, 819, 818, 723, 715, and 617 air filters. EHS observed ice build up in refrigerators in rooms 823, 826, 819, 715, and 617. Corrective action: PIC will have housekeeping keep up with the changing dates noted on each filter

Inspector note: EHS osberved varying degree of stains and hairs on all rooms observed. Several rooms had missing mattress protectors and/or mattress protector broken. Corrective action: EHS suggested housekeeping wear hair nets and to ensure mattress pad are included and are intact

Violation: Where laundry facilities are provided, they shall be separate from other facilities, of sound construction, and shall be kept clean, and in good repair. Inspection note: EHS observed excessive accumulation of fabric softener on the majority of fabric softener barrels. Corrective action: PIC will have barrels cleaned and ensure accumulation of such is properly addressed at time of spills

EHS observed facility unable to provide copy of gas-fired equipment inspection. Engineering manager stated believes they would be contracted to have completed inspection with Hayes. Contacted Hayes to obtain. Violation: facility shall have inspection conducted at least annually and shall provide proof of inspection upon request. CA: obtain inspection report

EHS observed ice build up in refrigerators in rooms 213, 214, 216, 435, 510, 534, and 709. Violation: all appliances must be maintained in good repair and clean. CA: ensure to defrost refrigerator's at frequency to prevent ice accumulation

EHS observed dust and or mold like accumulations on air returns and air vents in rooms 301, 305, 313, 407, 413, 510, 517, 534, 624, 613, 611, 607, 709, 716, 721, and 805. Violation: all appliances and accessories must be maintained in good repair and clean. CA: clean returns and vents at frequency to prevent accumulations

EHS observed debris or hairs on bedding of all rooms observed. Violation: all bedding must be kept clean in good repair, and vermin free. CA: debris check dryers and static blockers; hairs recommend hair restraints in laundry and housekeeping

EHS observed mattress pads missing in rooms 213, 215, 216, 301, 313, 435, 517, 534, 624, 613, 611, 607, 716, and 805. Violation: mattress pads shall be provided in all guest rooms. CA: ensure to place mattress pads in rooms prior to occupancy
