EHS observed a clear bottle containing a chemical without any label to identify what it is.
CA-PIC needs to make sure that employees are labeling all chemical bottles with its common name.

EHS observed the 3 compartment sink had a leak. When you turn the water on it goes out onto the floor.
CA-EHS informed PIC about the puddle on the floor from the 3 compartment sink. EHS explained that they need to get maintenance to service the sink to stop it from leaking.

EHS observed the floor needed to be cleaned there was trash present on the floor.
CA-PIC informed EHS that they just had a event the day before.

(a) Good Repair. All physical facilities shall be maintained in good repair.
(b) Cleaning, Frequency and Restrictions.
The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing.
If present, playground equipment and associated areas shall be maintained in a clean and sanitary condition. In addition, a cleaning and sanitizing plan along with cleaning and sanitizing records for playground equipment and associated areas shall be maintained within the food service establishment for review upon request by the Health Authority. Further, such cleaning and sanitizing plan for employees to follow in the event of encountering vomitus or fecal matter shall be included and submitted at the time of permit application as specified in DPH Rule 511-6-1-.02(1)(c). Violation: 1. A handwashing sink shall be equipped to provide tempered water at a temperature of at least 100ºF (38ºC) through a mixing valve or combination faucet. Pf Inspector note: EHS observed hand washing sink not sufficient in reaching required 100F. Corrective action: PIC will have maintenance resolve issue.

Violation: Each handwashing sink or group of adjacent handwashing sinks shall be provided with: 1. Individual, disposable towels; Pf 2. A hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.Pf Inspector note: EHS observed no hand drying provision within facility. Corrective action: PIC will ensure maintenance has facility stocked at all times.
Regulation: 511-6-1.05(3)(a) - cooling, heating, and holding capacities (pf)
Violation: Equipment for cooling and heating food, and holding cold and hot food, shall be sufficient in number and capacity to provide food temperatures as specified under DPH Rule 511-6-1-.04. Pf Inspector note: EHS observed cold holding unit and walk-in refrigerator out of temperature of 41F or below. Corrective action: PIC will have maintenance resolve issue.

Violation: 2. Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. Inspector note: EHS observed all sinks (3C, prep, and hand washing) not sufficient in reaching required peak hot water demand. Corrective action: PIC will have maintenance resolve issue.
