At time of inspection, the hot water temperature on the second and third floors exceeded 120 degrees F (Room 210 = 132 degrees F; Room 311 = 136 degrees F). PIC states she will have the water heater thermostats adjusted and will ensure that hot water temperatures do not exceed 120 degrees F. PIC states new water heaters were recently installed.

4/6/2026: PIC states all thermostats have been lowered to below 120 degrees F. This violation has been corrected

At time of inspection, facility does not have gas-fired equipment inspections on-file. PIC states a new dryer was installed recently, as well as new water heaters. PIC is contacting the company who installed them, to request the inspections. Provided PIC with a copy of the gas-fired equipment inspection form.

4/6/2026: PIC states she has been unable to get the company she contacted to visit the facility to perform the inspections. PIC states she will contact Dekle Plumbing, to request that they come out to perform the gas-fired equipment inspections. I will check back with the PIC in one week (on 4/13/2026)

At time of inspection, observed debris in the microwave in Room 311. PIC asked housekeeping to go clean the microwave thoroughly

At time of inspection, observed lack of anti-slip mat/coating in the tub in Room 311; also observed flaking mats in other rooms inspected

At time of inspection, observed worn/damaged refrigerator/TV cabinets in rooms inspected (the cabinet in Room 325 is particularly damaged). PIC states renovations are planned, and furniture will be replaced at that time

At time of inspection, observed soiled ceiling of the microwave in Room 302. Ensure that all surfaces of interior of the microwave are cleaned and sanitized

At time of inspection, observed several severely worn items of furniture in rooms inspected (specifically, the refrigerator cabinet in Room 325 is severely worn and damaged). PIC states facility is ordering new furniture, but is awaiting purchase approval

At time of inspection, observed stained pillow protectors in rooms 220 and 309. COS: PIC removed the stained items and asked housekeeping to replace them with clean covers. PIC states she will remind housekeeping to thoroughly inspect linens for stains/rips/holes

At time of inspection, observed absence of anti-slip mat/applique in the tub/shower in Room 309. PIC states she will replace the mat in the tub/shower

At time of inspection, observed stained pillow protectors In Room 316. COS: PIC had housekeeping staff replace the stained pillow protectors

At time of inspection, observed worn furniture; specifically, the nightstand in Room 121 was observed to be worn and peeling around the bottom. CA: PIC stated new furniture is expected to be ordered December 2023. Also observed several grains of rice in one of the dresser drawers in Room 225. COS: PIC cleaned out the drawer immediately

At time of inspection, observed stains on pillow protectors in all rooms inspected. COS: PIC removed the stained pillow protectors immediately and replaced with a clean pillow protector
