Rooms 102, 103, 104, & 105 had dirty dresser drawers (observed debris/build-up) and/or drawer bottoms falling out.

CA: CA: Charlton EH instructed PIC to clean more frequently and to repair the drawers.

Rooms 103, 104, & 105 had dusty air filters in the A/C unit.

CA: Charlton EH instructed PIC to clean more frequently to prevent dust build-up

Observed a leak from the washing machine where the outlet hose goes to the drain.

Room 105 had a non-functional bathroom fan.

Observed living and dead German cockroaches in two rooms inspected. Room 107 had five (5) roaches total: 4 in the refrigerator and 1 in the drawer. Room 114 had one (1) dead roach in a drawer.

CA: Charlton EH ordered PIC to obtain pest control services and to keep the invoices per TA permit requirements

CA: Clean up/discard unnecessary items.

Fence surrounding the burned down section of motel must be repaired (put back in place) by 11 July 2025. If not, TA permit will be suspended until fence is put back in place to prevent entry into an unsafe building.

CA: Given until 11 July 2025 to correct and forwarded to Charlton County Building Inspectors

Bathroom vent fans dusty in most rooms inspected. Room 106 had a non-functioning bathroom vent fan.

All rooms inspected had severely stained pillows and mattresses. No protectors are on any of the pillows or mattresses.

CA: Told PIC, again, to replace stained pillows and mattresses. Also told PIC that mattress and pillow protectors are required

Most current inspection not posted. Operator attempting to conceal grade by displaying 2022 inspection where they received a 93 instead of the most current inspection with an 85.

Discussed with PIC about permit and inspection display as it is not on display to guests checking in and must be put in window as TA does not allow guests in the lobby

Observed multiple rat droppings on top of hot water heater as well as in surrounding utility room and in the corner behind the dryer in the laundry room. Observed a German cockroach between the door sill of the freezer in room 106. POC: Facility told to get a licensed pest control company on contract

Facility has no annual gas inspection letter on file. POC: Inspector left a gas-fired inspection letter to be completed by applicable professional certifying gas-fired water heater is in compliance and functioning properly

All mattresses and pillows in rooms inspected had stains and/or rips. Box springs had stains and/or rips. Some linens had stains. CA: Told PIC that these items with stains and rips needed to be replaced

Observed dirty linens physically touching clean linens, no physical separation of clean and dirty linens. CA: Told PIC that dirty linens must be kept separate from clean linens
