Observed the facility to not have the annual gas fired equipment form.

RCA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed metal tongs in the individual room ice buckets to not be washed, rinsed, and sanitized between each guest stay.

RCA: Water glasses, ice buckets and other multi-use utensils provided for tourist’s use in a guestroom or dwelling unit shall be washed, rinsed, and sanitized, in a 3-compartment, warewashing sink dedicated for that purpose only, after each occupancy and as needed during occupancy. In lieu of sanitizing ice buckets, a sanitary, food grade, plastic bag, large enough to line the bucket and overlap the top edge may be provided for use by a single occupant

Observed the comforter cover to not be clean in room 321, and 415.

RCA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens
