Upon entering the facility, observed both back doors open with no protection from pests entering into the facility during operating hours. Front door has air curtain being used; however, no other doors have any methods of protection from outdoor contaminates. CA: PIC shall fix to protect from outside contamination. RCA: If the windows or doors of a food service establishment, or of a larger structure within which a food service establishment is located, are kept open for ventilation or other purposes or a temporary food service establishment is not provided with windows and doors as specified under paragraph 1 of this subsection, the openings shall be protected against the entry of insects and rodents by:
(i) 16 mesh to 1 inch (16 mesh to 25.4 mm) screens;
(ii) Properly designed and installed air curtains to control flying insects; or
(iii) Other effective means.

Observed no handwashing sink sign at the handwashing sink in main kitchen. CA: PIC shall post handwashing sink sign at sink. RCA: A Sign that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees.

Observed inspection report posted on board within kitchen that was not visible to the public. CA: PIC shall post inspection report in public view. RCA: The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
15C - nonfood-contact surfaces clean

Observed food debris build up on gaskets in reach in coolers underneath the prep top coolers. Observed food debris build up on the fan of the walk-in cooler. CA: PIC shall deep clean all equipment and non-food contact surfaces of equipment. RCA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
