28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed mattress pad staining in rooms 121, 231, 333, 431, 436, 413 Observed stained mattresses in rooms 333, 436, 413. Observed bed apron in room 244 to have heavy staining. RCA: PIC was informed and stated these would be cleaned and replaced. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed build-up on ice baffles withing all ice machines on floors 1,2,3,4 on both sides of the hotel. RCA: PIC was informed and reminded to ensure baffles are maintained and check weekly for build-up on baffles. Dispensing ice machines shall be stored in a clean place and shall be kept clean
32 - separate, structurally sound, clean, and in good repair: dryers vented and adequate lighting

Observed heavy lint build up throughout the entire laundry room facility on the walls, and ceilings. Observed multiple missing ceiling tiles and broken ceiling tiles within the entire laundry facility. RCA: Where laundry facilities are provided, they shall be of sound construction, and shall be kept clean, and in good repair
