Observed slime build-up on the baffle of the ice machine. CA: PIC shall burn ice and deep clean ice maker RCA: Equipment food-contact surfaces and utensils shall be clean to sight and touch.

Observed three missing ceiling tiles in main kitchen near walk-in cooler. PIC indicated that the contractor repairing the walk-in cooler damaged the tiles and have not been replaced yet. RCA: Replace tiles with smooth and non-absorbent tiles. 511-6-1.07(5)(a),(b) - Good Repair, physical facilities maintained; Cleaning, Frequency & Restrictions, cleaned often enough to keep them clean (C) (5) Maintenance and Operation.

(a) Good Repair. All physical facilities shall be maintained in good repair.

Observed light bulb in walk-in cooler not operating. Lighting in walk-in cooler was not to the correct intensity. RCA: 511-6-1.07(3)(f) - Lighting Intensity, adequate in food prep, storage & service areas (C) (f) Lighting Intensity. The light intensity shall be:

1. At least 10 foot candles (108 lux) at a distance of 30 inches (75 cm) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning;
