Observed walls and ceilings of walk-in cooler to contain black mold like substance all throughout. Observed coolant hose for walk in cooler to have white moldlike substance on it. Observed top of food racks in walk in cooler to have moldlike substances on it. CA: PIC reached out to maintenance and custodian to clean. RCA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Observed ceiling tiles and exhaust vent in the main kitchen having accumulated dust/debris.

RCA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing.

Observed inoperable vent hood in kitchen.

CA: PIC states a work order has been submitted and equipment requiring use of vent hood is not being used and will remain non-operable until vent hood is repaired.

Observed walk-in cooler not capable of maintain required temperature of 41F or below. Ambient reading was 47F. CA: PIC discarded all TCS food items (see notes for details) that were not within the appropriate temperature range. Maintenance was called to service the unit. Facility has a secondary walk-in that will be utilized until repaired.
