2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
Observed no verifiable employee health policy (EHP) forms in place for the person in charge (PIC). Blank forms were given during last inspection and are currently on site but no signed copies were able to be provided. This was discussed with PIC in last inspection. Food employees and conditional employees must be informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. COS by signing EHPs for the PIC.
4-2B - food-contact surfaces: cleaned & sanitized

Observed waffle maker with significant food debris being stored as clean. Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS by cleaning equipment.
11D - thermometers provided and accurate
Regulation: 511-6-1.05(2)(x) - tmd, located in refrigerators/ hot holding (pf, c)
Observed no analog thermometer in reach in cooler. Must have a thermometer in cooler placed close to the door.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(d) - displaying of the inspection report (c)
Observed an inspection report from 2022 with a score of 100 (A) posted in public view. The previous inspection was a 95 (A) and was located behind the 100 score. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. COS by discarding 100 (A) score and replacing with the most recent score
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(e)1,2,4 - equipment, utensil, linens, stored 6" off floor in clean, dry location (c)
Observed clean dishes stored on counter of 3 compartment sink directly next to hand washing sink, exposing the dishes to splash contamination. This was a violation on the previous inspection. Must install a splash guard in between 3 comp sink & hand sink or change the floor of washing dishes from right to left.
