Observed CFSM certificate not posted in public view. An original CFSM certificate shall be posted in public view at all times. Stated to PIC to post the CFSM certificate in public view.
2-2A - management knowledge, responsibilities, reporting

PIC could not demonstrate in a verifiable manner that food employees are/were aware of their responsibilities to report foodborne disease illnesses and symptoms. An employee health conditional agreement form(s) shall be maintained and produced upon request of the Health Authority. Stated to the PIC to obtain and produce employee health conditional form to the Health Authority upon request.
4-2B - food-contact surfaces: cleaned & sanitized

Observed food contact surface of the interior of the bulk ice machine s shoot in the main kitchen stored as clean storing ice with severe black mold-like substance. Equipment and food contact surfaces of utensils shall be cleaned at any time during the operation when contamination can occur. Stated to the PIC to increase the cleaning frequency of the bulk ice machine.
