Observed no employee health reporting document onsite. Advised PIC employees shall report illness in a verifiable manner.
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
Observed no vomit/fecal written procedures at time of inspection. Advised PIC food establishment shall have written procedures.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed multiple food items left uncovered at cold well at front-food service not in use. Advised PIC food shall be stored covered, from overhead contamination. COS: PIC covered food items.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed interior of microwave (top portion) at the front food service area stored with accumulation residue on food contact surface, Advised PIC equipment food contact surface shall be cleaned per manufacturer's recommendation as often as necessary.

Observed cardboard lining multiple shelving units (nonfood contact surface). Advised PIC nonfood contact surfaces of equipment shall be constructed of nonabsorbent material.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed staining ceiling tiles in lobby area. Advised PIC physical facilites shall be cleaned as often as necessary.
