Regulation: 511-3-5-.19(2)(e ) - (e) soap dispensers for providing either liquid or powdered soap shall be provided at each lavatory. the dispenser shall be of all metal or plastic. no glass shall be permitted in these units.
Observed no soap in restrooms. COS: PIC placed soap.

Observed Chlorine level reading at 0 PPM. Advised PIC the proper chlorine level shall be between 2-10ppm.
Regulation: 511-3-5-.23(5)(b) - (b) during operation, the ph is less than the minimum or more than the maximum levels allowed in dph rule 511-3-5-.l7.
Observed the ph level reading at 7.0ppm. Advised PIC the proper ph level shall read between 7.2 and 7.8ppm.
