Observed multiple items at grill prep reach in freezer next to fryer stored uncovered. COS: PIC placed covers on items. CA: All food items shall be covered as to prevent cross contamination.

Observed door of reach in freezer near hand wash sink at grill prep in need of repair. Advised to repair. CA: All equipment shall be kept in good repair.

Observed cracked floor tiles at grill prep area and near 3-compartment sink and prep area table and bulk food storage area in facility. Advised to repair. CA: Floors shall be kept in good repair so they are smooth, non-absorbent, and easily cleanable.
