Observed heavily soiled spoon stored with clean utensils near 3 compartment sink. COS: PIC cleaned and sanitized. CA: Food contact surfaces shall be kept clean.
11A - proper cooling methods used: adequate equipment for temperature control

Observed containers of TCS food items stored tightly covered cooling in walk in cooler. Advised PIC to keep loosely covered. COS: PIC covered loosely CA: Use proper cooling method to rapidly cool TCS food items.

Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed door to warmer cabinet not sealing properly. Also observed walk in cooler door not sealing. Advised to repair and take out of use until repair. CA: Equipment shall be kept in good repair.
