6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed multiple TCS items at the self-service cold well (shredded cheese, boiled egg, and spring mix) and self-service ice bath (sliced cheese, sliced lettuce, and sliced tomato) cold holding in the temperature danger zone above 41 degrees F. TCS foods shall cold hold at 41 degrees F. or below. COS: PIC place items in WIC for proper cooling.
18 - insects, rodents, and animals not present

Observed visible light entry at the back door of the facility. Outer openings of a food service establishment shall be protected from the entry of pest and rodents. Stated to PIC to place weatherstrip on back door.
