Advised PIC to thoroughly clean furnishings in guestrooms listed

Advised PIC beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Advised PIC to thoroughly clean mattresses and mattress covers in guestrooms listed

Observed staining on light shield in guest toilet room 201; observed missing light shield in guest toilet room 210, 109, 106, and 104; observed hairs on towel in guest toilet room 104; observed shower curtain rod in disrepair in guest toilet room 109; observed stains on shower curtain in room 201.Advised PIC that toilet rooms and its fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Advised PIC to thoroughly clean and repair all guest toilet rooms

Observed visible light entry at the front door of guest rooms 201, 210, 109, 106, and 104. Advised PIC openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors. Advised PIC to place weatherstrip on doors to guestrooms listed

Observed outside ice machine's interior panel on the 1st floor stored as clean with brown/black mold-like substance. Advised PIC that all ice machines shall be kept clean and in good repair at all times. Advised PIC to thoroughly clean the interior panel of ice machine

Observed no cold water at faucet available for room 107. Advised PIC Advised PIC Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F

Observed one dead roach on bed of room 208. Advised PIC Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional

Observed no pest control records onsite at time of inspection, upon request. Advised PIC A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. The record may be made by the permit holder or provided by the applicator, and shall list the following information: (a) amount and concentration of product used; (b) name of product used; (c) date and location of application; (d) application method; (e) pest targeted; and (f) name of applicator

Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program
