Observed screen door at back door of facility with visible light entry. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: Solid, self-closing, tight-fitting doors. COS: Had PIC close back door of facility.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used

Observed cardboard lining shelf in Reach-In-Cooler, Juice stored on top. Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Observed grease accumulation build-up on fryer basket on non-food contain surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed stains on ceiling tiles in need of cleaning. All physical facilities shall be maintained in good repair. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
