Observed facility without a gas-fired equipment inspection report. Advised PIC all gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available

Observed staining and hairs on couch cushions in rooms 129, 123,and 207; observed debris and mold inside of coffee maker in 207 and 123; observed staining on curtain in rooms 233; observed rusting on the interior and exterior of mini refrigerator in room 223; observed interior of freezer with debris in room 329; observed microwaves with food debris and rusting in rooms 129, 207, 235; observed buildup on room in room 123. Advised PIC all furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Observed staining on bed linens, mattress covers, and pillows in rooms 129, 123, 235, 310, and 329; observed staining on mattress in beds and pull-out couches in rooms 310, 325, 123, and 207; observed debris on pull-out couch in rooms 207 and 310. Observed missing mattress on pull out couch in room 129. Advised PIC Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed staining and hairs on towels in multiple bathrooms in rooms 207, 233, 235, 129, and 123. Observed rusting and debris in showerhead in rooms 129, 123, 235, and 233. Observed bathtub with debris in room 235. Advised PIC Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair

Observed ice machines on the 1st and 3rd floor stored as clean with black mold-like substance. Advised PIC the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Advised to increase the cleaning frequency of ice machines

Observed guestroom water temperature exceeding 120 degrees F. in rooms 123 (122F), 223 (122F), 227 (123F), 302 (123F), 309 (122F), and 311 (124F). Advised PIC that n all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120.0F. Advised PIC to decrease water temperatures in guestrooms

Observed debris and rusting in microwaves in rooms 223, 302, 309, 311, and 321. Observed stains on couch cushions in rooms 223, 227, 302, 309, 321, and 311. Observed holes on couch cushion in room 311. Observed material peel on desk chair in room 302. Observed debris in freezer portion of refrigerator. Advised PIC that all furniture and accessories in bedrooms shall be kept clean and in good repair at all times. Advised PIC to thoroughly clean and replace furniture items listed

Observed stains on mattresses and mattress covers in rooms 302, 309, and 227. Also observed stains on pull out mattress covers in rooms 227, 309, 311, and 123. Advised PIC that all bedding including mattresses and mattress covers shall be kept clean at all times

Observed debris in tub in rooms 309 and 311. Also observed rusting on showerhead in rooms 223 and 227. Advised PIC toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Advised PIC to thoroughly clean all toilet rooms and its fixtures

Observed rusting in microwaves in rooms 119, 330. Also Observed tears on small stools in room: 217. Advised PIC that all furniture and accessories in bedrooms shall be kept clean and in good repair at all times

Observed stains on mattresses for rooms: 238 and 111. Also, observed stains and hairs behind headrest area on bedskirt for rooms: 333, 217. Advised PIC that all bedding including mattresses, mattress covers, and bedlinens, shall be kept clean at all times

Observed guestroom water temperatures exceeding 120F in rooms 225 (123F), 102(123F), 102 (124F), room 111 (124) and room 113 (122F). Advised PIC that all guest rooms water temperatures shall not exceed 120F

Observed rusting and debris in microwaves in rooms 225, 102, 113, 233, and 103; Observed staining on couch and couch cushions in rooms 102 and 113, Advised PIC that all furniture and accessories in bedrooms shall be kept clean and in good repair at all times

Observed mattresses, bed linen, and bedskirt with severe staining in rooms 225, 102, 113, 111, 103. Also observed stains on pull out mattress and mattress covers in room 113, 103. Advised PIC that all bedding including mattresses, mattress covers, and bedlinens, shall be kept clean at all times

Upon arrival, observed 2022 tourist accommodation permit posted in public view. Advised PIC that a current tourist accommodation permit shall be posted in public view at all times. Advised PIC to obtain 2024 permit

Observed ice machine on the 1st floor stored as clean with mold-like substance on white panel. Advised PIC that water supply and sanitizing equipment including ice machines shall be maintained so they area cleaned all times

Observed damaged and stained ceiling tiles through laundry area of facility. Advised PIC that ceilings shall be kept clean and in good repair. Advised to replace ceiling tiles

Observed multiple chemical bottles on housekeeping cart unlabeled with chemical's common name. Advised PIC that chemicals placed in a multi-use bottle shall be labeled with the chemicals common name. Advised PIC to label all chemical bottles

Observed guestroom water temperatures exceeding 120 F in rooms 303(123F), 209 (122F), 211 (122F), 127 (121F) and 105 (123F), 125 (122F). Advised PIC that all guestroom temperatures shall not exceed 120F

Observed debris and rusting inside microwave in rooms 105 and 303; observed stains on couch cushions in room 303, 240, 209, 211,127 and 125; observed severe staining on lamp shade in room 125. Advised PIC that all bedroom furniture and accessories shall be kept clean at all times

Observed stains on bed skirts, mattress pads, mattress, and observed hairs on bed linens in room rooms 303, 240, 211, 209, 125, and 105. Advised PIC that all bed skirts, mattress pads, mattresses and bed linens shall be kept clean at all times

Observed 2023 Tourist Accommodation permit not prominently displayed in public view. Advised PIC that a current Tourist Accommodation permit shall be posted in prominent view at all times

Observed crack in bathtub in room 303; observed staining in bathtub in room 340, 209, 211, 127, and 125; observed minor stains on towels in room 125 and 209. Advised PIC that all toilet facility fixtures and rooms shall be clean and in good repair at all times

Observed 1st and 3rd floor ice machine with black/brown mold like substance. Advised PIC that all ice machines shall be clean at all times

Observed multiple unlabeled chemical bottles stored on housekeeping cart. Advised PIC to label chemical bottles
