Observed CFSM certificate not posted in public view upon arrival. Advised PIC The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times and shall be made available for inspection by the Health Authority. Advised PIC to post an unexpired CFSM certificate in public view.
2-2A - management knowledge, responsibilities, reporting
PIC could not demonstrate in a verifiable manner that food employees are/were aware of their responsibilities to report foodborne illnesses and symptoms. Advised PIC food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. Advised PIC to obtain an employee health policy and to sign conditional agreement forms.

Observed the most recent inspection report not posted in public view. Advised PIC that the most recent inspection report shall be posted at all times. Advised PIC to obtain the most recent inspection report and post in public view.

Observed the current Food Service Establishment permit not posted in public view. Advised PIC that the current Food Service Establishment permit shall be posted in public view at all times. Advised PIC to obtain the 2025 Food Service Establishment permit and post in public view.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed ceiling tiles near the pizza oven with staining. Advised PIC The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Advised PIC to thoroughly clean ceiling tiles.
