No hot water observed at handsink. PIC turned Hot water off. CA: A handwashing sink shall be maintained so that it is accessible at all times for employee use. COS: PIC turned hot water on to handsink.
4-2B - food-contact surfaces: cleaned & sanitized

Observed food contact surfaces of knives with encrusted residue stored with clean utensils. Advised PIC all food contact surfaces shall be clean to sight & touch.

Observed front door open upon arrival. Advised PIC outer openings of the facility shall be protected at all times. COS: Front door was closed.

Observed multiple roaches in kitchen. Last pest control visit was 11-18-25. Facility shall be free of pest. Advised to contact pest control company to ask for retreatment.

Observed wall shelves and tops of standing equipment and deep fryers with build-up on the sides in the main kitchen area. Advised PIC nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
