No mattress pad observed in room 308 and on one bed in room 316. Stained mattresses observed in both rooms. Beds in room 316 had no blankets or covers, with one bed having no pillows. C/A: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase

Hallways very dirty, trash bags on floors and trash cans overflowing. Observed dirty linens on floors in common spaces. C/A: Grounds, including spaces beneath buildings and trailers, shall be kept clean and free of litter

Vents not operating in rooms 306, 308, 316. C/A: Toilet, lavatory and bathing facilities shall be mechanically ventilated

Soap not provided in rooms 308, 316. C/A: Unused, individually wrapped soap or liquid soap dispensed from approved containers shall be provided in guest rooms

No toilet tissue provided in rooms 308, 316. C/A: Toilet tissue shall be provided in a dispenser at each toilet at all times

Floors, walls, and ceilings in hallways and all rooms inspected were stained, peeling, cracked, and soiled with several types of dirt buildup. CA: Floors, walls, and ceilings shall be maintained in good repair

Observed inadequate and non-operating lighting in rooms 308 and 316. C/A: All rooms shall be well lighted. When natural light fails to provide sufficient illumination, evenly distributed artificial light shall be provided to maintain a lighting intensity of not less than ten foot candles at 30" above the floor

Observed cockroach activity in rooms 416 and 201.

Observed hallway ceilings with water damages and heavy stains. All rooms inspected had bathroom ceilings with mold-like stains.

Observed in room 404 mattress with hole, stains in bedding in both beds. Observed no mattress topper on the second bed.

Observed floors, walls and ceilings in all rooms and hallways with damages, stains and holes. Observed missing ceiling tiles in hallways and in laundry room of first floor.

Observed bathroom sink water above 120f in rooms 209(125f) and 105(130f).

CA: In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F

PIC unable to provide gas fired equipment inspection for 2023. Last inspection from 3/8/22.

CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available

Observed guests' trash stored on top of cleaning cart without proper disposal.

CA: Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room

Observed unlabeled chemical bottle stored in housekeeping cart.

CA: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents
