6-1A - proper cold holding temperatures

Observed cold held TCS foods at temperatures greater than 41F in the front food service prep top cooler and counter top. CA: Cold hold TCS foods must be maintained at 41F or below. COS:Prep top cooler: Provolone, blue cheese, ranch, and tartar sauce 41F, rice discarded.
6-1B - proper hot holding temperatures

Observed chicken nuggets on the front food service counter hot holding at a temperature less that 135F. CA: Hot held TCS foods must be maintained at 135F or above. COS: 3PM label placed on the food to discard.
2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
PIC unable to provide a verifiable manner in which food employees have been informed of their responsibility to report to the person in charge (PIC) as it relates to foodborne illness.

Observed mop bucket with mop inside sitting in front of the handwashing sink in the main kitchen. CA: A handwashing sink shall be maintained so that it is accessible at all times for employee use. COS: Mop bucket remove.
